Fill in the form completely after reading the notice below.

Registration-form

• The items with ∗ are mandatory.
• When you apply for the oral or poster presentation, do not forget to give the title and abstract as well. Note that the number of oral presentations is limited and the poster presentation will be automatically alloted to those who are not selected for the oral presentation.
• The banquet fee is 3,000 yen per person.
• If the confirmation of registration is not sent to your email address soon after submission or if you want to change the contents of registration, send an email to us with your name, affiliation and other information put in the email content. Do not submit a registration form again.

Deadline for submission

• Oral contribution: January 31, 2012
• Poster contribution: February 20, 2012
• (Banquet: February 20, 2012)

Financial support of travel expenses

The budget is very limited and not all the requests will be satisfied.
